Our practice has a patient portal that you can use to view summaries of your appointments, and to communicate with your doctor. If you would like to log in, please provide us with an e-mail address during your next visit.

 

Introduction

We are committed to maintaining Your confidence and trust with respect to any information, recorded in any form, about an identified individual, or an individual whose identity may be inferred from the information. Storage and access to medical records, E-consultations, and other services offered to Users are collectively referred to herein as the "Service" or "Services." We intend to protect the Personal Information stored on the Site and this privacy statement describes how We intend to do that. BY SUBMITTING PERSONAL INFORMATION TO US, BY USING OUR SITE, AND BY USING OUR SERVICES, YOU AGREE THAT WE MAY COLLECT, USE AND DISCLOSE SUCH PERSONAL INFORMATION IN ACCORDANCE WITH THIS PRIVACY POLICY AND AS PERMITTED OR REQUIRED BY LAW. If You do not agree with these terms, do not provide any Personal Information to Us. If You refuse or withdraw Your consent, or if You choose not to provide Us with any required Personal Information, We may not be able to provide You with products or services that can be offered on our Site. Minors (as defined under the laws of their jurisdiction or residence) are not eligible to register for, use, or purchase the products or Services available on our Site. How to Access, Modify or Delete Your Personal Information.

We encourage You to login to Your member account to review Your Personal Information to ensure that Your Personal Information is as accurate, complete, and current as possible. You can access, update, correct, or modify Your Personal Information by logging on with Your User name, password and 4 digit PIN. If You request that we remove all of Your Personal Information stored on Our Site, we will promptly do so to the extent possible. Once Your Personal Information is removed, however, You will not be able to use many of the Services available on Our Site unless you register. Please remember that Our healthcare records storage program is subject to certain terms and conditions and the rights to access, modify, or delete those records are subject to those terms and conditions.

Cookies

Since deleted data will not be restored, You may want to print information before deleting it.

Providing security for the Personal Information Users submit to Our Site is one of Our top priorities.
